Our Wraparound Support Framework
We connect individuals in recovery, veterans, at-risk youth, and local families to essential community services through a centralized case management model.
- Supportive & Transitional Housing Coordination: Placement assistance for supportive housing, sober living environments, and transitional residential facilities tailored to individuals completing rehabilitation or leaving unstable living conditions.
- Eviction Prevention & Rental Assistance Navigation: Direct linkage to local New York emergency rental grants, tenant advocacy programs, and public housing assistance to keep individuals and families safely housed.
- Essential Documentation & Benefits Advocacy: Hands-on assistance securing critical identification documents (birth certificates, state IDs, Social Security cards) and enrolling in Medicaid, SNAP, SSI/SSDI, and public assistance programs.
- Community Reentry Support: Specialized housing and resource navigation for justice-involved individuals transitioning back into the community.
- Referral & Resource Network: Connection to food pantries, emergency shelters, legal aid, healthcare clinics, and utility assistance throughout New York.
